To access the quizzes (which backdate to 10/01): You'll need the session name. I'll put the names for every quiz in the same format so all you have to do is choose the specific date you want.
The format is: SI quiz (here you will enter the 4-digit date mm/dd e.g. for Oct. 01 you'll enter 10/01)
E.g. for the quiz from Oct 1st the session name is
SI quiz 10/01

Please use one of these methods to print off a copy of the SI survey and fill it out- this is how they evaluate how I'm doing so if you like me or hate me let the ARC know by filling this out and bringing it in to me ASAP.
