After reviewing the educational PowerPoint on Central Line Removal, take the mandatory post-test by CLICKING HERE. . If you have trouble accessing the test, please click on the link below for the "Central Line Removal Post-Test". You must achieve a passing grade of 5/5 to pass. You may retake the test once. After you pass the test CLICK HERE to access the Critical Elements Checklist for Central Line Removal. Print a copy. Fill in your name and signature. You must have it validated in the clinical area by either your PCUM, APCUM, or preceptor. Then give it your PCUM or APCUM to be filed in your record. Retain the second copy for your personal records.
