Information & Requirements for Current Members of NHS
If you are a current CSH NHS "member-in-good-standing" as of the final roster in the spring of 2009, here are the forms, documents and procedures you need to protect your status as an active member. Be certain to read the bylaws you are acknowledging before you complete the necessary documents. Remember that some of these forms require a parent/guardian acknowledgement/signature even if you are already 18. As for this year's due dates, please check the QUIA calendar for specifics on firm deadlines. Note that in most cases late fees apply - in others, your membership may be affected via probation or dismissal. If you have questions regarding your current status, please see Mr. Johnston to double check our most current records and membership roster.
CSH NHS Chapter Bylaws
Update Your Registration, Pay Dues and Acknowledge Bylaws for 2009-2010!
Download and print a copy of this year's updated bylaws from the link posted below. When you've reviewed the requirements for 2009-2010, download, print and complete the registration form from the link below. Prepare cash or write a check to "CSH NHS" for your annual dues of $15. Place the completed form in an envelope marked clearly on the outside with your name and the phrase "Annual NHS Dues." Be sure to seal it for your protection! Bring your envelope to Mr. Johnston in CSH Room 121, or to the "NHS Drop Box" in the CSH Guidance Office. Please follow these guidelines carefully as NHS cannot be responsible for lost, stolen, or unmarked/unlabeled envelops. A late fee of $5.00 will be charged for payments processed after the deadline.

Website details at QUIA have changed as of June 6th, 2009, so as part of the registration process for 2009-2010, you are required to register an up-to-date e-mail account with the organization for the purpose of communication - - even if you did so last year!! This information will be kept confidential and will only be used for official NHS business. If you do not complete this important step, NHS cannot be responsible for a lapse in communication regarding important deadlines, meeting dates, community service opportunities, etc. Though every effort is made to communicate on campus at CSH, we cannot guarantee that all information is shared effectively via announcements, KDGN, or scrolling announcements unless you register for e-mail promptly. Should your e-mail change, you are solely responsible for updating the website with correct e-mail info. The QUIA directions for registering your e-mail are included here for you to download and use.

Orders for 2010-2011 NHS Merchandise (Pins, Patches, Shirts & Graduation Stoles!)
NHS Members! You can order ALL of your merchandise in one simple order! Orders for official NHS pins, patches, t-shirts, polos, and graduation stoles can be made together. Download, print and complete the order form from the link below. Total your order, and prepare cash or write a check to "CSH NHS." Place the completed form in an envelope marked clearly on the outside with your name and the phrase "NHS Order." Be sure to seal it for your protection! Bring your envelope to Mr. Johnston in CSH Room 121, or to the "NHS Drop Box" in the CSH Guidance Office. Orders can be picked up on campus from Mr. Johnston once all items are in and accounted for. Seniors, graduation stoles will be held until May for pick-up so that NHS can verify that all annual requirements (GPA, dues, hours, orders, etc.) have been met before graduation.

NHS Community Service Hours & Procedures 2010-2011
Each year, members of NHS at Carroll are required to complete (10) hours of community service expressly for NHS to fulfill the annual membership requirement. Please understand that you cannot complete hours for NHS while completing hours for your degree plan or another club/organization. These hours are a formal way for you to show friends, family and your Southlake community that you follow and respect the NHS expectations regarding society and service. Because our organization is so large, no events will be made mandatory. This affords you the flexibility to choose an activity that suits your personal needs and schedule. These hours can be completed individually or by participating in an NHS-sponsored/NHS-affiliated event.
As you complete your hours, be sure to use the correct form to record your progress - you can download and print the form from the link below. Hours completed for your graduation requirement via the Carroll Community Service Board (yellow cardstock forms) CANNOT be used, reclaimed or re-assigned for your NHS annual requirements, as these are separate entities entirely.
DO NOT TURN YOUR HOURS IN TO THE COMMUNITY SERVICE BOARD OR YOU MAY LOSE THOSE HOURS FOR NHS!! (Obviously, several students assumed that, "Hey, they're hours, so they go to the same place, right? WRONG!!)
When you have completed your hours for the year, you may turn in your form to Mr. Johnston in CSH Room 121, or to the "NHS Drop Box" in the CSH Guidance Office. Strict deadlines apply to your membership status when these forms come due (. . .sometime in mid-April - final deadline to be posted via website), so don't be late!
NHS Community Service Hours Form
NHS Tutors Always Welcome!
Each year, CISD students require additional support outside the classroom for the courses they face. Because academic scholarship is a large part of our chapter's basic creed, we supply a contact list of members to district campuses' guidance offices for the purpose of providing tutors. Students tutoring for NHS receive community service hours for each hour signed for by the parent of the student receiving the instruction. NHS tutors can complete the entire annual requirement by providing this service, and may choose to turn this into a profitable part-time job once annual commitments have been met! Parents and students alike are reminded that NHS tutors serve no other purpose than that of academic assistance and instruction, and that they should NOT be expected to provide additional mentoring services above and beyond normal academic assistance. NHS tutors have the right to refuse service if a convenient schedule cannot be worked out for both parties. This opportunity is a YEAR-ROUND need with help needed desperately at the beginning of the year as students adjust to new workloads and expectations - as soon as NHS members register, a list for each discipline area will be created and submitted to campus guidance offices. The finalized lists are usually compiled by early to mid-November each year.
The NHS Application & Induction Process in Carroll ISD
The official application and induction process is administered on the Carroll High School campus via Dr. Becci Rollins in the CHS Guidance Office. The process begins with an administrative review of all sophomores who qualify by the end of the fall semester of the sophomore year. A candidate’s cumulative GPA (freshman through current year) must be a 93.0 or higher in order to qualify. If you've met GPA guidelines, you'll receive an official invitation on-campus or in the mail via Dr. Rollins sometime in February that guides you through the application process. It’s the responsibility of the applicant to reply accordingly in order to be considered. Once you've received that invitation, you'll need to complete the application posted on this website by the published deadlines sometime in March in order to be considered. No applications will be processed beyond the official deadline as outlined in your official chapter invitation. Additional qualifications regarding community service are outlined on the application, and all sections must be completed along with the essay before you can be considered for induction. When the faculty council has approved your application for membership, you'll receive a notice on-campus that guides you through the induction ceremony (planned for sometime in March or April), and the annual registration process for our CSH chapter. Once you have your certificate of membership from the chapter, you may begin to use the official CSH NHS designation on university or scholarship applications. At that point, all annual expectations regarding NHS membership, including dues, community service hours, and other chapter requirements will be expected of you according to chapter guidelines and due dates - in other words, you must actively maintain the membership you have just earned. How to have a successful year in the chapter will be discussed in fuller detail in a chapter meeting on campus following the induction ceremony.
NOTE: Because CSH NHS does only one formal induction ceremony each year in the spring, you cannot join mid-year if you miss this opportunity, or simply decide not to join at this time. Continue to maintain your GPA and fulfill your normal community service requirements for graduation, and you'll receive another chance to in the fall of your junior or senior year.
Already a junior or senior? The application process for current juniors and seniors who have recently qualified for the induction process with a 93.0 cumulative GPA will begin with on-campus invitations delivered by Mr. Johnston and the NHS officers in late October. Completed apps for new juniors and seniors will be accepted sometime in mid-November, with the final inductees posted to this website sometime in early December. As soon as inductees are notified of their status as a new member to NHS, they may begin to use the NHS designation on official correspondence to colleges and universities. A formal induction ceremony will be held at CHS sometime in the spring to honor the accomplishment you have achieved. NHS Application for Membership
New to CISD, But Already an NHS Member?
It's simple! Have the chapter sponsor at your old school send us a letter on school letterhead, or have the sponsor send us an email verifying your member in good standing status, and we'll add you to the roster! The following information needs to be included in the memo: Your Name, Date of Induction, Membership Status at Time of Withdrawal, and School Sponsor's Contact Information (Name, School Address, Phone Number, E-Mail, etc.). All you need to do beyond that is abide by all CSH NHS bylaws, pay your annual dues, and begin fulfilling your annual community service commitment. Any of our sponsors, officers or Senior Senate members would be only too happy to guide you through the process as you familiarize yourself with names and faces in your new Carroll Dragon home!
The NHS Annual Scholarship Program
Each year scholarships are awarded to a select number of graduating NHS seniors at Carroll Senior High. The size and number of NHS scholarships depends upon dues and fundraising conducted within each school year. In the past, the CSH chapter has awarded anywhere between (7) $500 scholarships up to as many as (10) $600 scholarships.
Seniors are enthusiastically encouraged to apply as the application consists primarily of an essay. The essay prompt is kept secret until applications are released to the applicants according to the schedule posted to this website’s calendar. Recipients are selected by an anonymous panel that, in the past, has consisted of NHS chapter sponsors, CISD teachers, and CISD administrators. The identities of the applicants are kept secret during the selection process to ensure that awards are being made fairly, and without personal bias.
Senior NHS members interested in being considered for an NHS scholarship must follow the guidelines as set forth by the chapter regarding all procedures and deadlines. Dates for signing out an application from Mr. Johnston, and deadlines for submitting completed apps and essays are posted on this website in advance. No late submissions will be accepted for this prestigious honor. Communications regarding the scholarship selection process will be limited to NHS e-mails, scrolling announcements and CSH hallway posters. Please understand that no letters will be sent home to notify seniors or their parents that apps are available. All scholarship recipients will be notified in person, on campus sometime in early May. A final list of recipients will be posted on this website prior to graduation in June.

NHS Officer Election & Honor Senate Selection Process
Each year in the spring, NHS conducts officer elections and Honor Senate selections to provide the general membership with a viable and active student leadership core. The officers and the Honor Senate are charged with providing leadership by setting annual chapter goals, by reviewing, evaluating, and maintaining chapter policies, and by motivating the general membership to participate in a variety of NHS initiatives from community service to fundraising.
Officers are elected by the their NHS class peers sometime in April; to be clear, graduating senior members of NHS do NOT participate in officer elections for the new academic year. Although officers must be seniors for the year that they intend to serve, Honor Senate members are selected from both incoming junior and senior classes. Candidates for officer and the Honor Senate complete the same application according to chapter guidelines in order to be eligible. A candidate for officer not elected is automatically considered for placement in the Honor Senate, though this is NOT a guarantee for final selection. Officer candidates must sign up with Mr. Johnston as they turn in completed applications for the purpose of composing an official ballot. Officer candidates must also attend a mandatory meeting with Mr. Johnston prior to the campaign period to verify their candidacy and intent to run. Campaigning for office consists roughly of two weeks of hallway poster campaigning conducted according to chapter guidelines, with elections held before school, at lunch, and after school on a single school day soon thereafter.
Honor Senate members are selected by a faculty advisory council soon after elections are over, once the officers have been named. The criteria for Honor Senate selection by the faculty council is designed to provide a quasi-representative body of students who represent a cross-section of the CSH campus with respect to academic disciplines, extracurricular activities, and off-campus obligations. Applications for officers and Honor Senate members are made available according to the website calendar usually sometime following the spring break holiday in March.
