* 1 folder with brads (Per.1=red; Per.4=green; Per.6=purple; Per. 7 yellow; Per.9=blue
* $.50 for a new disk (school policy-no outside disks)
$.25 for a used disk
* 1 ream of paper
* Internet Form SIGNED
