MMS School Counseling Department - Parent Referral

Dear Parents/Guardians:

Please allow me to formally introduce myself. My name is Heather Cody and I am the new School Counselor here at Manteno Middle School. My role at MMS is to help aide in the development of academic, vocational, and social needs of the students through collaboration with school staff, parents, and community members. I look forward to reaching these goals by working individually, as well as in groups, and class discussions.

I have attached a Needs Assessment that you are free to fill out which can help me to gain an understanding of you and your student’s specific needs in an effort to support you, the parent/guardians. You are welcome to fill out this form and return to either my mail bin, the front office, or please feel free to stop by my office. I also will have an electronic version available on the Manteno Middle School Website.
