- Effort. I work hard during math.

- Focus. I center all my attention at math on a task, problem, or goal.

- Initiative. I follow through. I start math right away, and I complete my work.

- Listening. During math I give all my attention to the speaker to hear and follow directions. I also listen to my classmates when they are explaining how they think.

- Speaking & contributing. I raise my hand and contribute ideas. I use an appropriate voice when sharing during large groups, with small groups, and during quiet periods during math.

- Responsibility. I take charge of my learning. I do the right thing in math class. When I need help, I ask for it.
|
- Homework. I complete homework, and I hand it in on time.
|
- Cooperation. I work well with others to do an activity or complete a task during math. I am willing and cooperative when working with many different people.

- Facts. I know my multiplication facts through 12 x 12.

- Attitude. I like math. I do my best and give my best. I come to class ready to listen, to contribute, to do the work, to learn.

- Considering my homework completion, my focus, my effort, my understanding of math, and the assessments I've taken in math, I think I deserve the following grade: E= exceptional, above grade level; S = Successful, I am successful in math; P = Partially successful, I am partially successful in math but could improve what I know and my work completion; N = I should improve, I need to change my math work, work completion, and understanding of math
