Player Pre-Academy Self Evaluation - Fall 2009

Please answer these questions as honestly as possible. This form will give the coaching staff and you a starting point from which we can establish a plan that best fits your needs.

Your answers will not be shared with anyone else. We view honest self-reflection as an important tool in trying to be a better basketball player. So take your time, and be honest.
