The Performance Evaluation of the Senior Choir Director (SCD)--April 2011
Making evaluations on purely a "yes" or "no" survey is not recommended, but know it can be overturned (and fixed within moments) if you wish. I'm going on a five point scale with "1" being the "strongly agree". I have also taken all of the original questions and reworded them into statements. All of these statements begin with "The Senior Choir Director (SCD)..." Though I wrote all 30 questions here (and one optional question at the end), this should be edited to no more than 20 if asking the congregation or the choir for assistance in the survey. It is currently set for names to be optional, but that can be changed as well.
