Please take a moment to accurately evaluate your intern on his/her performance for the six week grading period. Honest feedback will provide the student with an accurate picture of his/her performance and identify areas where improvement is needed.

      Dependability: Intern accepts responsibility, follows instructions and can be counted on to complete tasks. Cleans up after all jobs.   
      Attitude: Intern displays a positive demeanor and does not bring personal problems into the classroom. Courtesy, tact, and manners are displayed.   
      Personal Appearance: Jeans are only worn with black BCTAL shirt; street clothes never include jeans; name badge is worn daily; all grooming is appropriate for the classroom.   
      Initiative: Intern takes on work without being told. Intern follows through with tasks.   
      Attendance: Intern is present on all scheduled days. Rare absences are reported to you in advance. Intern is always prompt, never tardy.   
      Work Accuracy: The intern is almost always accurate on tasks; requires the minimum amount of supervision.   
      Judgement/Common Sense: The intern uses good judgement in making decisions.   
      Industriousness: The intern uses time efficiently and effectively.   
      Knowledge/Willingness to Learn: Knowledge of the job grows daily through participation in the classroom or asking questions when unsure about duties.    
      Cooperation: Intern works well with students, teachers, and all school staff members.
