Dear Participant, to ensure that we have the most up to date information, we are conducting a brief needs assessment to find out what prevention curriculums, if any, each of you are utilizing in your classrooms. This survey is going to be used ONLY for district/grant planning and is not a reflection on the individual completing the survey.We'd like to find out what other resources you might need.
