This survey will be used to gather baseline data in applying for a technology grant for our school. Please respond honestly. When appropriate to the question, use 1 for low and 5 for high.

    Level 1 - I do not understand what responsible use means.
    Level 2 - I take care of the equipment I use and leave it in the ready condition for the next user.

    Level 1 - I rarely use a computer at school, home or elsewhere and am not familiar with basic skill operations.

    Level 3 - I can open, use, and close programs on my own and can use more than one program at a time.
    Level 4 - I learn new programs and discover additional program features on my own or through help from my teachers and other students to complete assignments.

    Level 1 - I do not save any documents I create using the computer.
    Level 2 - I select, open, work with and save documents on different drives (my I:/ drive or floppy disk, etc.)
    Level 3 - I create my own folders to keep files organized and and can navigate from Courses/Teacher Templates to my own folder locations in my I:/ drive or other disks.
    Level 4 - I use the school's network effectively by moving between drives and folders to retrieve files and manipulate them for class assignments.
