Training Needs Survey
Instructions:
This survey is focused on identifying the training needs for professional and leadership skills for fiscal year 2012. Your participation and responses will be confidential.
This survey has two parts. The first part should be completed by all employees. The second part should be completed only if you are a supervisor, manager or leader.

Part 1 - Questions 3 through 8
Part 1 is focused on identifying the skills that are most important to achieving your goals and objectives. When completing part 1 think about your job responsibilities, goals and key initiatives for fiscal year 2012 and then identify your training priorities using the scale below. A rating of "1" signifies your lowest training priorities and a rating of "4" signifies your highest training priorities.
