Understanding Evidence Based Practice in Case Management

Dear Case Managers,

You are receiving this survey for QI purposes so that we can gain a better understanding of your perception of evidence based practice in service plans. If you have worked here for greater than 2 years, we ask that you complete this survey. If you have not worked here for greater than 2 years, then you do not need to answer this survey. However, if you worked at another Advantage agency as a case manager during the past two years, we do ask for your participation.

Please rate these items as honestly as possible. If you are unsure about how to answer an item, go ahead and answer it but then explain in the space below each item. Your results will be confidential, will not be reported individually and will only be presented in grouped information. We would like you to place your name on the survey so that I can track who has answered it and who has not.
