Dear Case Managers,

This is a survey to assess OU Case Management's technology needs. For the purposes of this survey, when we state documentation we mean any patient identifying information. It is required that all case managers complete this survey and it is also required that you place your name on this survey. Please complete this survey by July 12, Wednesday at noon.
