Information & Instructions
- Take the test and evaluation by clicking on
the links below. Both are
mandatory.
- You must receive a grade of 80% or greater in
order to pass the course. You will have 3 opportunities to pass the test.
- REMINDER: Completion of the e-Learning course alone will NOT VALIDATE
or CERTIFY the learner for in-hospital direct patient care for accessing
ports.
- Notify Staff Development Online by email, fax
or by phone when you have passed the test. Click on "View Profile" which is located at the top of this page. Refer to the instructions on
the Port Program Home Page and General Information links.
Printable Forms and Instructions:
- Print a copy of the skills competency
form for port accessing and the instruction sheet for the live patient experience. Bring the skills competency form to both your skills class and live
patient experience.
