83-year-old female with prior authorization request for right total knee arthroplasty (CPT 27447).
FOV/LOV January 11, 2016. The patient was evaluated for pain in the right knee after bumping her knee and creating a bruise. A subsequent ultrasound revealed no evidence of DVT. She had been intermittently limping and has difficulty working outside the home. She takes Tylenol PRN pain .

On examination of the right knee, there is market valgus alignment with medial lateral laxity. There is antalgic gait and restricted range of motion.

X-rays show severe tricompartmental osteoarthritis with complete loss of the lateral joint space with erosive changes and vagus deformity.
