68-year-old male with prior authorization for right total hip arthroplasty (CPT 27130).

FOV 11/4/2015. Patient reports severe pain in the right hip for the last three months without history of trauma and with complaints of restricted range of motion and difficulty ambulating. There is no prior history of surgery or treatment for the hip.

Physical examination shows a Trendelenburg gait with significantly restricted range of motion in internal and external rotation. Unsteady Trendelenberg gait with use of a cane.

X-rays show advanced severe degenerative osteoarthritis with complete loss of joint space.
