Physician 1st Quarter 2016 Spine Surgery IRR - Case Study #5
58-year-old male with prior authorization request for electrical osteogenesis stimulator (CPT E0748). Patient has been complaining of neck and radicular pain into both upper extremities and has had extensive conservative management which has failed. Diagnostic studies and clinical evaluation are suggestive of severe degenerative disc disease at C5-6 and C6-7 with severe neural foraminal stenosis and large disk/osteophyte complex, which is consistent with his clinical examination. He has no prior history of previous cervical spine surgery and no significant comorbidities. A two level ACDF was subsequently approved. The treating surgeon recommended a bone growth stimulator "to increase fusion rates due to the multilevel nature of the surgery."
