Blue Shield California Commercial - Use BSC epidural for back pain as criteria. Lumbar Interlaminar Epidural Steroid Injection – 62311 46yoF with request for repeat caudal or interlaminar epidural steroid injection. She is S/P L4–5 hemilaminectomy in 06/2013 and 11/2014. Patient received 70% relief of radicular pain from caudal epidural less than 2 weeks ago. She is still complaining of tingling in right foot after standing/walking. Physical examination of 04/25/2016 showed only intermittent pain/paresthesias in the lower extremities, no motor of reflex changes. MRI from 01/12/2016 shows L5–S1 moderate to severe central stenosis, no nerve root compression.
