Lumbar Facet/Medial branch nerve block – 64493, 64494, 64495
55yoF with request for a therapeutic facet/medial branch nerve injection. She is on Neurontin and has problems with walking, secondary to the low back pain. MRI was ordered. The patient has undergone medial branch nerve blocks on 04/21/2016 with an initial which was approved on 03/15/2016. Both had excellent pain relief.
On physical examination, she has spasm of the paraspinals with trigger points, decreased range of motion in flexion and extension with pain in extension and lateral rotation.
