59yoF with request for left sacroiliac joint radiofrequency ablation. The patient has low back pain S/P motor vehicle accident of 01/19/2015. She is S/P left sacroiliac joint injections of 09/25/2015, 11/18/2015, 12/15/2015, and 01/26/2016. She feels “a whole lot better,” but it only lasts 2 months.
Physical examination was unremarkable. Lumbar MRI of 04/04/2015 showed left convex scoliosis and spondylosis.
