57yoM with request for right C4–5, C5–6, and C6–7 medial branch nerve radiofrequency ablation. She has moderate and constant pain in the neck, radiating to the right shoulder/arm and bilateral hands to the thumbs. Right C4–5, C5–6, and C6–7 dual medial branch nerve blocks gave significant relief of the right neck pain. Cervical MRI of showed sever right and moderately severe left C5–6 foraminal stenosis. 10/2015 C5–6 interlaminar epidural gave relief for 2 days.

On physical examination showed moderate restriction of motion but otherwise unremarkable.
