74yoF with prior authorization request for total knee arthroplasty CPT 27447.

Patient has had right knee pain for 5 years with over 6 months of failure of conservative care, including PT, injections, and NSAID treatments which interferes with ADLs.

Physical examination reveals marked patella femoral crepitation with ROM and tenderness of the patellar region. There is normal alignment and ROM from 0- 120 degrees without instability.

X-rays show moderate patella-femoral DJD with good maintenance of the medial and lateral joint spaces.
