67yoF with prior authorization request for right total knee arthroplasty CPT 27447.

Patient has painful right knee for the last year and has had physical therapy and NSAIDs for the last 2 months without benefit. There have been no intra-articluar knee injections or use of ambulatory aids.

Physical examination reveals a morbidly obese female with BMI of 48 with antalgic gait and joint line tenderness with crepitation and restricted ROM from 5-100 degrees.

X-rays show advanced medial joint arthritis with Grade IV Kellgren-Lawrence DJD.
