70yoF with prior authorization request for left total hip arthroplasty CPT 27130.

Patient has had chronic left hip pain for the last several years, which is severe and aggravated by stair climbing and daily activities. Treatment has included physical therapy, medications and ambulatory aids for the last 6 months.

X-rays show advanced DJD left hip with sclerosis, periarticular osteophytes and cysts with obliteration of joint space.

Physical examination revealed ½ “ shortening of left leg with painful active and passive ROM which is restricted in all planes. Gait is antalgic.
