63yoM with prior authorization request for right total knee arthroplasty CPT 27447. Patient has longstanding chronic right knee pain for 5 years without history of trauma or previous surgery. He has had failure of extensive conservative care for 2 years including NSAIDS, brace, intraarticular steroid and viscosupplement injections and physical therapy. The injections only provided temporary relief. Physical Examination revealed mild antalgia without crepitation during ROM which is from 0-120 degees. There is only mild joint line tenderness and no instability or effusion. X-rays show moderate DJD with slight narrowing of medial joint space, no subluxation or osteophytes.
