67 year old male requesting for a repeat Cervical ESI.
Patient complains of severe neck pain radiating to the Right upper extremity.
Physical exam shows decreased sensation in the lateral aspect of the arm with positive Spurlings.
Patient is currently doing home exercises and aquatics.
Patient has had 3 ESIs, the first one done on 3/27/16. All three injections gave excellent relief
