Total Hip Arthroplasty-CPT 27130

64 year old M has complained of hip pain for one year and presented initially to his specialist 6 months ago. His symptoms include dull, achy pain in the right hip and groin with inability to stair climb. He can only walk 2 blocks without having to sit.

Treatment has included medications, physical therapy and ambulatory devices since the initial office visit.

Physical examination reveals BMI 30 with an antalgic gait and painful rotation with limitation of flexion to 80 degrees.

Xrays show joint space narrowing with subchondral cysts, sclerosis and large osteophytes of the right hip.

Surgeon is requesting to perform a right total hip arthroplasty.
