Total Hip Arthroplasty-CPT 27130

This patient is a 68 year old morbidly obese female with left hip pain for the last 3 months. There is no history of trauma. She has seen her family physician 3 months ago who prescribed NSAIDs and referred her to the orthopedist.

She has had injections into the trochanteric bursa x2 with good results but continues to have groin pain. She ambulates with a cane and has done physical therapy for the last 3 months without any other documented treatment.

Physical examination reveals a BMI of 47 with an antalgic gait and limited, painful hip ROM, especially with rotational maneuvers. There is no hip instability on examination.

Xrays show advanced DJD of the left hip with loss of joint space, sclerosis and large osteophytes.

Surgeon is requesting a left total hip arthroplasty.
