Pt c/o left side LBP. Denies any radiation, numbness, tingling. Pain present x 3 months.
PE: + pain is worse with extension and rotation of lumbar spine.
Pt has failed >6 weeks conservative treatment with Norco, Lyrica, and HEP
MRI shows facet arthropathy from L3-S1
MD is planning RFA if injection provides relief.
