C/O Hx diabetes with history of osteoporosis and has had chronic neck pain and radiculopathy resistant to all conservative care for over 6 months, including PT and injections.
PE: Physical Examination reveals findings consistent with Right C6 nerve root compression with 4/5 weakness and associated decreased sensation in C6 dermatome.
Imaging results indicate severe advanced degenerative disc disease with HNP at C5-6.ACDF has already been approved.
The surgeon is requesting an adjunct Electrical Spine Stimulator to assist with the fusion due to co-morbidities.
