Request for bilateral L 4 – 5 radiofrequency ablation of the medial branch nerves.
TX: She has had single medial branch nerve blocks which gave 60 – 70 % relief at 11 days.
PE: showed decreased range of motion.
Imaging showed lumbar spondylolisthesis
