Reason for test: Suspected Coronary Artery Disease (CAD).
Current S/S: dizziness, light-headedness, malaise/fatigue and weakness x unspecified duration. States she gets an occasional sharp CP which can go to right jaw and left arm. Does have occasional exercise but limited by back and hip pain.
PE: BP: 124/88 mmHg, PR: 104 beats per minute, WT:152 lbs., HT:5’2” ,BMI:27.79.
PMH: current smoker, GERD.
FMH: Mother – DM ,high blood pressure, heart disease, stroke.
Hx of Cancer: None noted.
