5. Do not record POA indicators in this document.
6. Be sure to record the MS-DRG as well as diagnosis and procedure codes.
7. If you have the ability, you may replace the table in this document with a screen shot from your encoder that includes the MS-DRG and your ICD-10-CM and ICD-10-PCS codes (instead of manually entering the MS-DRG, ICD-10-CM, and ICD-10-PCS codes into the tables). The screen shot must be clear and large enough to be easily read.
8. Diagnosis Coding (Z-Codes and External Cause Codes)

a)Do code long term use of medications, history of chemo, history
of radiation, and any other relevant medical and surgical history.

b)Do not code external cause codes.

9. Procedure Coding In addition to any OR, IR, cardiac cath procedures, and/or mechanical ventilation:

a)Do code CVCs, PICC lines, and midline catheters, fluoroscopy
performed during heart cath procedures, chemotherapy & anti-
neoplastic immunotherapy administration, vasopressor
administration, thrombolytic administration, new-technology,
CPAP/BIPAP except for newborn resuscitation, and dialysis.

b)Do not code transfusions, EEGs, imaging other than that
performed during cardiac cath procedures, administration of
medications other than those listed above, or fetal monitoring.

10.Be sure to save your document frequently so codes are not lost.
