CASE MANAGER REQUEST - V2

USE THIS FORM ONLY IF THE IT MEETS THE FOLLOWING CRITERIA
- Customers that are within warranty (MUST HAVE ESP/ADP IF OVER 1 YR COMPANY WARRANTY)
- Customers that have had difficulty getting a resolution through Tech Support (MUST have tech support case #)
- Tech support case # is not needed for Missing Software, TV Issues, assistance with ESP registration or refund
- If outside the 21 day return policy, customers can still be assigned a Case Manager if they have a tech support case # and have purchased through the HP HHO Store
- notes in TOMI the reason of submitting the form and what customer wants

EVERY INVALID REQUEST WILL BE ISSUED A CORRESPONDING COACHING OPPORTUNITY
