The Ascension Legacy CSM Readmission Reduction Steering Committee is seeking input from Primary Care Providers on types of information that should be included in the hospital provider discharge summary that would help to ensure continuity of care. The responses will be used to enhance the communication at discharge between providers to ensure a good transition for the patient. Thank you for your assistance.
