- For Tribal plans, the exam and lenses are covered in full once every 12 months from the date of service. The maximum is used for frames and contacts, which do not have a frequency. Lens options are available and paid under the plan.*

- The member can have multiple policies for themselves and for dependents and will always be covered on all of them.*

- For Non-Tribal plans, the exam and lenses are covered in full once every 12 months from the date of service. The maximum is used for frames and contacts, which do not have a frequency. Lens options receive a discount if they visit an EyeMed provider only and are out of pocket expenses.*
