Yearly Gift Survey

We have been given a budget from Corporate of $5 per quarter or $20 per year to be able to spend on each Colleague. We would appreciate your input as to how you would like us to spend it on you. A list of suggestions for your selection has been provided. There is room for your suggestions, also, recognizing that these may not always be able to be accommodated.
