UseManagement of IBS and patients who do not respond to usual interventions
ContraindicationsGlaucoma, Cardiovascular disease, Myasthenia gravis, Paralytic ileus, obstructive uropathy, severe ulcerative colitis, Reflux esophagitis, infants <6months, hepatic/renal impairment, CHF, Autonomic neuropathy, Prostetic hyperplasia
Side Effectsconfusion, constipation, heartburn, anaphylaxis, paralytic ileus, increased ICP, palpitations, tachycardia
Interactionsadditive anticholinergics, other orally administered drugs ( slowing motility of GI tract), antiacids, adsorbent anitdiarrheals ( decreases absorption of anitcholinergics), oral potassium chloride tablets (may increase GI mucosal lesions)
DoseAdults: (PO) 10-20mg 3-4x daily ( up to 160mg/day)
Nsg ImplicationsAsses for signs of IBS before and periodically during therapy, monitor I&O ( may cause urinary retention)
Patient TeachingInstruct the patient to take the medication as needed as prescribed (take missed dose as soon as remembered as long as it's not right before next dose, good oral hygiene to prevent dry mouth, avoid driving or activities requiring alertness until response to med is known, notify HCP immediately if eye pain or increased sensitivity to light occurs (emphasize routine eye exams), change positions slowly to decrease effects of orthostatic hypotension, avoid extreme temperatures ( may increase risk of heat stroke & decreases ability to sweat)
