PTA 259 CPI Criteria
For each question please fill in the numerical average % grade. You do not need to put the '%' symbol just the number. On the horizontal VAS each centimeter is worth 10%. The beginning of the VAS is 0% and the end is 100%. If your mark is on or about 3cm then your average is 30%.
Example for 'Average for criteria 3 & 8'. If your CI puts your mark for criteria 3 at 5cm and your mark for criteria 8 at 7cm the average is 6cm or 60%
