2019 - 2020 Good and Perfect Attendance

**Please submit this form electronically by Friday, May 29, 2020 with the current eligible employee names. Indicate “None” or “No Eligible employees” if there are no employees who qualify for Good or Perfect Attendance after Question #1. All schools must complete the form even if there are no eligible employees.

**The monetary bonus for Good and Perfect Attendance is from August 14, 2019 through June 2, 2020 for Assistants and July 31, 2019 through June 3, 2020 for Managers and Assistant Managers. Good attendance applies to those individuals who missed 2 days or less of work.
