      1 2 3     N/A
    Yes  No

  1. Is there anything that would cause you to remove your child from PATH that were COVID-19 related?

You might say "If masks aren't required, I would remove my child". Or you might say, "If masks are required, I would remove my child".

We will most likely be asked to follow the guidelines of the building we are in. Currently CEFC, our designated building is following CDC guidelines which will involve social distancing, masks, and some other items. It is all very fluid, even to the point of PATH not being allowed in the building. We will know more in July.

The purpose of this questionnaire is simply put to gather information anonymously to see how COVID-19 restrictions or lack thereof would affect our enrollments.

Here are the issues that could affect PATH:

    1. Masks
    2. Social Distancing
    3. Periodic cancellation of PATH and "Zoom" as a replacement
    4. Schedule interruptions (building closure, etc)

There are so many that I may not have thought of. Please take some time and share your thougths below. If you will "roll with the punches" you can just say that, too.

  1. This question in particular is about the PATH calendar. Here I would like you to opine about a few items. I will ask several questions in a row.

Are you open to another day for PATH if necessary? Do any days not work? We are not wanting to change, but our current facility may need us to do that to make things work.

  1. Are you supportive of using Zoom if necessary due to building closures?

  1. Here is your chance to indicate any other items of concern that would affect your enrollment at PATH. I certainly have overlooked things, so please do your best to think through "schedule" and "the day in the life of a PATH student".
