2020-2021 Good and Perfect Attendance

*Please submit this form electronically by Friday, May 28, 2021 with the current eligible employee names.

*The use of Personal Reasons Leave (SPR), COVID-19 Leave or hours taken off without pay (PLV), will not count for purposes of Good and Perfect Attendance if used before 10/9/2020.

*Indicate “None” or “No Eligible employees” if there are no employees who qualify for Good or Perfect Attendance after Question #1. All schools must complete the form even if there are no eligible employees.

*The monetary bonus for Good and Perfect Attendance is from October 12, 2020 through June 9, 2021 for General Workers and Cooks and October 12, 2020 through June 10, 2021 for Managers and Assistant Managers. Good attendance applies to those individuals who missed 2 days or less of work.
