2021 - 2022 Good and Perfect Attendance

**Please submit this form electronically by Friday, May 27, 2022 with the current eligible employee names. Indicate “None” or “No Eligible employees” if there are no employees who qualify for Good or Perfect Attendance after Question #1. All schools must complete the form even if there are no eligible employees.

**The monetary bonus for Good and Perfect Attendance is from August 18, 2021 through June 9, 2022 for Cafeteria Staff and August 4, 2021 through June 10, 2022 for Managers and Assistant Managers. Good attendance applies to those individuals who missed 2 days or less of work.
