Supplier Evaluation - M & B Products 2023 - Milk
The purpose of this evaluation form is to rate a supplier’s performance. However, this evaluation must take into consideration the “Supply Chain Issues” that have plagued the ability to secure food since the onset of the pandemic. Completion of this form will aid the Food and Nutrition Services Department in determining the quality of goods and/services purchased for the District.
