    Yes
    No
    I haven't noticed
    Other

  1. If you answered "yes" to the above, what were your reasons for doing so? (Select all answers that apply)
It is quicker/easier for me to purchase what I need when I see it rather than go through the ordering process with my school/district
I like to have control over exactly what I'm getting; often what is supplied by the school/district is not exactly what I want
I don't want to be questioned about how I am spending school/district funds
I am limited to how much I can spend on a monthly basis; this restricts me from purchasing a more expensive item using school/district funds
I see it as an investment; many of the items I can take with me if I transfer
    Other

  1. Would you agree, or disagree, that the supplies/materials/equipment that you obtain outside your school/district budget, contributes to student success? (a "3" would indicate you neither agree or disagree.)

    Strongly agree   Strongly disagree

  1. What has been your experience with I Love Schools.com (or other web sites that connect private donors to US schools and teachers to help provide supplies, equipment, materials they cannot obtain through normal channels)?
I didn't know these types of web sites existed
I know that these types of web sites exist but I have never investigated them
I have signed up on one of these web sites in the past but never saw any results
I like these web sites, I have received a few donations for requests that I have made
I LOVE these web sites, anything I ask for, I get!
