Please reflect on our first five weeks together and answer these questions honestly. If you entered Leadership AFTER the first week, then complete the survey based on the length of time spent in class. The survey questions are designed to not only provide me with important information from a variety of student/leader perspectives, but also illuminate you with information I feel is important. Your feedback may lead to changes in how we progress through this semester.

  1. Why did you join Leadership? Think back to the first time you considered becoming part of our school leadership team. Share with me what your desire was.


  1. Describe TWO passions you have for activities for the remainder of the year at our school. These activities could be something we have already done or tried to do (Puma Pass events), or something you would like to have us consider. Specifically highlight your ideal project for the remainder of this year.


  1. Special Part 1 "Asset Inventory" question. What occupations do your parents/guardians have (what do they do)? List their jobs and also their job titles. What can I do with this info? NETWORKING!! Think about opportunities for our school, students, classrooms, etc. if we have parents/guardians that have specific job functions or talents that would improve our learning environment.

    I have HIGH expecations for myself and constantly strive for consistency and gradual improvement in ALL areas
    My self-expectations are high but I allow some leeway as I am a struggling student-leaders
    I accept much deviation from the Advisors direction, and need counsel to guide me
    I tolerate my own not-performance and remain ambivalent towards my progress

DO NOT INFORM MR. HAYWOOD ON SOMETHING THAT IS OBVIOUS, INFORM MR. HAYWOOD ON WHAT YOU HAVE PERSONALLY OBSERVED THAT I MAY NOT KNOW ABOUT.


  1. Inform Mr. Haywood on any issue you feel is important for him to be aware of. These responses will be kept in confidence.


  1. If you went to yourhomework.com before 4:00 p.m.on Saturday afternoon there was important information I inserted into the assignment. What was that important information?


  1. What remaining Leadership event do you "Own"? This means that YOU are the primary person responsible for Chairing the event. YOU are the Commissioner of the event. i.e. Marvin OWNS the Puma Pass Party, Monica OWNS the Club Rush, and Karyll OWNS the Underground Dance. Sarah owns the Fashion Show. Who owns the Halloween Rally?


  1. What would you like to own and what is your plan for getting your item on the agenda? I did not advance a SINGLE pink sheet this week. Either we're getting close to advancing all of our Leadership events for the entire year, or we fell down on Pink Sheets this week. This must be rectified by the end of this coming week!


  1. Indicate SPECIFICALLY what assistance you need in order to bring your "Imagination" to life? How will you manufacture the "Time" needed to realize your vision?


  1. Hey! How well are our Fall sports teams doing? Are we effectively communicating their games, win record, etc? Who are some of our athletes? Why don't I see any personalization of names on our walls? What can you think of to provide more "school spirit" in this area?


  1. Indicate necessary supplies you feel we need to stock up on for upcoming events because without them we will be compromised in our ability to conduct effective operations. Who should be in charge (responsible) for taking this list through the ordering process?

  1. If you participated in the Earthquake drill last Spring, what can you remember was an area of improvement that we should talk about (perhaps with Mr. A) prior to this weeks drill.


  1. Oh, almost forgot. Special Part 2 "Asset Inventory" question. What unique talent do you posses that I may not be aware of (or someone else in Leadership for that matter) that might be beneficial to our class. For instance, do you know how to edit video using a PC? Or, are you gifted with writing copy - "how to" information or directions - like how to use the sound system, for instance. Or, maybe you are handy with tools, making things, putting large "stuff" together for a dance, or play, or maybe a fashion show. What can you do?


  1. Alright, final response. So now you can see there are a number of "areas of concern" I have on a go-forward basis with our class. Too many students are arriving late, not enough of the class appears to be on-task some of the time, we are not utilizing our resources to their best advantage (wasting blue tape), and we have a number of activities planned for the next four weeks, if not the remainder of the semester.

    Indicate up to five (5) students that you feel, based on your impression of what makes an "ideal" Leadership student, who are not making their BEST efforts in our class. Be specific with the students name and what you have personally observed that you want to make me aware of. The purpose of this question is two-fold: 1) inform the teacher because I simply cannot be in all areas at one time - if I am in the garage and one or more students are outside room 427 sitting on the floor doing very little I cannot see that. If there is a student in our classroom working on a non-Leadership activity I cannot tell that from how I utilize my Advisor time with the multitude of students who need information (Mr. Haywood, Mr. Haywood, Mr. Haywood, etc) and #2) as Leaders you will be faced with the unpleasand task in your futures of "trimming the rose bush so the beautiful blossoms will emerge in the Spring", this sometimes means you have to remove a friend because they're simply not showing the dedication and comittment you are.

    Your responses will be culled (look it up in the dictionary) and discussed with the Executive Committee later in the week.
