Weekly Participation Self-Evaluation for Studio Classes

Every Friday (or late Thursday) all Studio students need to fill out this self-evaluation form to grade their effort for the week. This weekly self-evaluation participation grade will have a large effect on each student's effort grade. Please answer questions honestly and rate your effort for each question on the grading scale (A, B, C, D or F).
