Homework Club
You are receiving this survey because you have one or more students who were enrolled in the after school SUN Homework Club. You have been provided a list of the names of the students who were in HW Club in a separate email. Please refer to this list when answering the questions below. HW Club began in mid-April and ended last Thursday. This survey will be used to help us understand how Homework Club may have been beneficial to your students and how it can be improved for next year. Please answer all questions with as much detail as possible. This information will be used to support funding for future programs like Homework Club and after school tutoring programs.

- Did any of your students (from the HW club list you were emailed) demonstrate improvement in the following areas:
1) Turning HW in on time
2) Completing all parts of weekly/daily HW
3) Accuracy and neatness
4) Increased understanding of key concepts introduced in HW
If so, please list student and area of improvement. Please be as detailed as possible.

- Was the coordinator (Krista Kelley) available to discuss student needs or issues concerning assigned homework? Were you aware of how to contact the coordinator if you needed to (email, phone, etc)?

- Students who participated in HW Club Tues-Thurs were encouraged to set a daily goal for completing assigned homework during HW Club (approx. 45 min in length). Students who completed daily goals were allowed to play Homeworkopoly, which is an incentive-based game that requires students roll dice and move around on a game board. Certain spots on the board are marked as prize and incentive spaces. Each time students complete their goal, they have a chance to win prizes and incentives. Do you feel that offering incentive-based games, like Homeworkopoly is an acceptable way to motivate students to set goals and complete work on time? If, not can you offer other suggestions for motivating students?

- Currently, Homework Club consists mostly of students who either have siblings in later SUN classes or who need to stay late because of transportation issues. One possibility for improvement with HW Club next year is that enrollment in HW Club would be based on teacher and parent recommendations. This would mean that HW Club would potentially be devoted to students who are recommended by parents and teachers. Would you be open to the possibility of recommending students whom you feel would benefit from HW Club each trimester? This means that you would be able recommend students in your class, who may need to work on specific skills or simply need a quiet space to complete their work. Please respond below.
