The purpose utilizing the QUIA/WEB format is to assist us in identifying area(s) of need with the TLC program participants.
The TLC participant mentored requires a single record for each mentoring session with a TLC mentor. It is not necessary to reply to each of the domain statements, only those that are applicable to your mentoring session with which you are reporting.
